FixedHeader: Row-height mismatch between leftmost column & right columns; excessive <td> height

    2 issues arise when using the FixedHeader add-on with a fixed leftmost column and 1 or more fixed header rows:
    -The height/positioning of rows is mismatched between the leftmost column and all columns to its right
    -The <td> height is excessive, given the maximum required height across all <td> instances

    Below are samples of output:
    http://107.21.250.122/1-th.htm: 1 x <th> element in the <thead>
    http://107.21.250.122/8-th.htm: 8 x <th> elements in the <thead>
