Is it possible to swap in a different pager at a given break point? I can probably figure out the break point logic, but I could not find anything in the API for modifying the pager after the init.

Currently no - the paging control cannot be altered after the initialisation is complete. It could possibly be hacked in, but there is no public API for that I'm afraid.
