Custom Export Excel Add Image and Text in top (header)

Hello dear i need an export Excel file from my current datatable, but i want add a custom header excel and adding image

this is a sample report i need when export to excel,
please see my attachment, there have an origanal export from dattable, and another report view i need,
please give me a sample code, how to make report like this, which image and had a custome header
