This is a CSS issue where two CSS libraries are displaying the buttons. I don't immediately see the specific issue. Typically with Bootstrap its because both
dataTables.bootstrap.min.cssandjquery.dataTables.cssare being loaded. I'm not sure which of your CSS files are conflicting.Since you are using Bootstrap I would recommend downloading/using the DataTables Bootstrap support libraries.

Thank you @kthorngren, but there is no "Bootstrap support library" on that page, the only options related to Bootstrap is the option of "Styling". I selected the Bootstrap styling and used the generated CDN, the duplicated order icon is gone, but the order icon also changed to the "Bootstrap style" (the wireless signal strength indicator) no longer the triangle.
Poor choice of wording :-) You found what I was referring to. And yes the sorting icons are different like you described.
