The template layout is actually up to you. The documentation here describes Editor's field structure, which will be used inside the template where you tell the fields to go, but the actual structure and layout of the template is whatever you want it to be.

I think an example of using Bootstrap would be helpful and an explanation of where to add those bootstrap classes. Can you add them right to the div with the custom tag? I tried that and that did not work.

No - attributes from the custom tag aren't used at all I'm afraid.
