There is a live example here (apologies for the text at the top of the example - it is incorrect and will be fixed with the next release).

There are also a number of examples on the dependent() documentation page at the bottom.

The PHP part is simply a case of querying the database to get the values you want based on the information in your form. It is highly dependent upon your exact configuration, but really all you need to do is query the database and return JSON in the form described on the dependent() page.
