Best way to create two lines using a DB data line

Hi all

I need to show two lines with information from a customer. These information come from a DB, for each customer I want to create two lines. These two lines will be export to an excel as one line.

After read a lot of posts (split, grouping, child rows, etc..) I still can't know how to do this.

Which are the best way to do this? There are?
